First VisitBring all the questions you would like to discuss and we will be happy to address all your concerns. A consultation fee will be collected and filed to your insurance plan for reimbursement / deductible. During your first visit, each patient will be asked to complete minimal paperwork and we will make copies of both your dental and medical cards. If you would like to complete some of your paperwork prior to arriving at our office, please feel free to use our online Patient Registration Form. |
